Neighbourhood Teams

The Neighbourhood Team is a dedicated service providing home visits for housebound patients or patients experiencing great difficulty attending the practice. Key areas of care include wound and leg ulcer management, chronic disease management, terminal and palliative care and the promotion of continence. The Neighbourhood Team also advises carers and teaches patients to manage their own health needs whenever possible.

Health Visitors

A health visitor is a nurse who has undertaken further training in child development and health promotion. They are closely involved in the health care of patients, particularly children and their families

Macmillan Nurse

The Macmillan nurse is a trained expert in the care of the terminally ill and the bereaved. Please ask your doctor if you feel they may be able to help.

Physiotherapists

Physiotherapy is available after referral by the doctor and can be arranged at Chippenham Community Hospital.

Community Midwives

Midwife means "with woman". Midwives care for the family throughout pregnancy, labour, birth and postnatally handing care over to the health visitor when the baby is about 10 days old.

Ante natal clinics are offered at Malmesbury Primary Care Centre on Monday, Tuesday, Thursday and Friday mornings and at Chippenham Maternity Unit on Wednesday mornings.  Postnatal visits take place in the afternoon.

GP Registrars

In addition to providing general medical services the practice also undertakes continuing education and training of GP registrars and other health care professionals working at the practice. GP registrars are fully qualified doctors who have chosen general practice as a career, and who will be gaining valuable experience by being based at our practice. They provide care of the same standard provided by the other doctors. Occasionally GP registrars are required to video record their consultations-however patients full approval will be sought beforehand and patients should not feel obliged to agree if they do not wish their visit to be recorded.
