Dear Patient,
I wonder if you would be kind enough to help both the Practice and our Patient Participation Group (PPG) better understand the needs and expectations of our patients. The Practice list size has risen to just over 14,500 patients, and whilst we cannot satisfy every patient all the time, it would be good to know more about what patients think we do well, or indeed could do better.
What will be involved?
We fully appreciate your time is precious, so we plan to keep matters short and to the point. Rather than trying to entice patients to meetings we plan to develop an email forum or a “Virtual Patient Reference Group”.
The main role of the group is to give us feedback through a survey. It will ask patients a series of simple questions about how we are doing. We will make paper versions available for those who do not have email access.
We want to pose the right questions, so the group will be asked to tell us what questions we should be asking. We will then publish the survey and then the results along with our proposal to improve the areas it highlights as in need of change. You will have the chance to let us know if you agree with our plans. At the end of the year we will publish the results of our achievements.
We will probably contact you two or three times a year for very brief requests or online questionnaires (totalling about 15 mins max per year) and of course to share with you all the results and findings .

Please be assured the information you give will always be anonymous and never linked to your name or medical records. We plan to start as soon as we get a sufficient number of email addresses to make the exercise valid.
